Giving feedback

Providing high quality care to you and your whānau is our priority, so we’d like to hear how we did, good or otherwise. You can tell us during your appointment, use the form above and choose Feedback, or email us at support@practiceplus.nz.

Making a complaint

If something hasn’t gone well, we want to know so we can put it right and learn from it. Email us at support@practiceplus.nz with “Complaint” in the subject line, or use the form above and choose Complaint or concern. It helps if you can include:

  • The patient’s name, and your name if different
  • The date of the appointment
  • What happened and when
  • What you’d like to happen as a result
  • A contact phone number
  • Any relevant correspondence

What happens next

We’ll acknowledge your complaint in writing within 5 working days. Within 10 working days of that we’ll let you know our decisions, or if we need more time to look into it. If that’s likely to take more than 20 working days, we’ll tell you. We’ll then let you know the outcome.
